How to Get a Library Card

General Requirements

A valid library card or a government-issued photo ID must be presented in order to borrow materials from the Kingman-Millcreek Public Library (KMPL).

Library card registration is limited to one card per household. Children are not eligible to receive a library card.


Adult Registration (Age 18 and over)

Applicants must apply in person at the Kingman-Millcreek Public Library and present acceptable Proper Identification at the time of registration.

Individuals who own property within the KMPL library district but reside outside the state of Indiana must provide proof of property ownership, such as a tax bill or deed.


Proper Identification

Acceptable forms of identification must include a photo ID. Examples include:

  • Valid Indiana Driver’s License
  • Valid Indiana State Identification Card
  • Valid U.S. or other government-issued photo identification (such as a passport, military ID, or permanent resident card)
  • Valid driver’s license or state-issued ID from another state

If the photo ID presented does not show a current address within the KMPL library district, the applicant must also provide one item from the Proof of Residency list below.


Proof of Residency

Documentation must include the applicant’s name and a current address. Acceptable forms include:

  • Computer-generated bank statement issued within the last 30 days
  • Computer-generated utility bill, credit card statement, or medical bill issued within the last 30 days
  • Medicaid or Medicare benefit statement issued within the last 30 days
  • United States Postal Service change-of-address confirmation (P.O. Boxes are not accepted as residential addresses)
  • Apartment lease signed within the last 30 days
  • Property tax receipt issued in the applicant’s name

Minor Registration

Library cards are issued only to individuals eighteen (18) years of age or older. Minors are not eligible to obtain a Kingman-Millcreek Public Library card under any circumstances.


Library Card Fees

Library card fees are determined as follows:

  1. Residents of Kingman and Millcreek Township – No charge
  2. Residents of townships without a contracted library service – Fee determined annually by the Library Board of Trustees
  3. ________ teachers and students who are classified as non-residents – Issued a library card at no charge for educational use

Individuals who do not reside within the service area may obtain a Kingman-Millcreek Public Library card by paying an annual non-resident fee of $70.00. The card is valid for one year and provides full borrowing access to materials owned by the Kingman-Millcreek Public Library.

The Public Library Access Card (PLAC) is a statewide library card program established by the Indiana General Assembly in 1993 pursuant to Indiana Code 4-23-7.1-5.1. The PLAC program allows an individual cardholder to borrow materials directly from participating public libraries throughout the State of Indiana.

A PLAC card must be purchased through an individual’s home library. Eligibility is limited to Indiana residents who hold a valid library card issued by an Indiana public library.

The cost of a PLAC card is seventy dollars ($70.00) per year, as established by the Public Library Services Committee of the Indiana State Library Advisory Council. The PLAC card is issued for individual use only, is non-transferable, and is valid for a period of one (1) year from the date of issuance.

A patron’s home library must be included on the reciprocal borrowing agreement. Patrons are required to have an active library card from their local library and maintain good standing on that account. Once these conditions are met, the patron may receive a library card that provides borrowing privileges limited to materials owned by the Kingman-Millcreek Public Library.
