Requests for Data
This form is to be used to request data from the Indiana Department of Health Division of Vital Records primarily in support of research to advance public health. We support the research of students and student projects, graduate school capstone projects, doctoral theses, universities conducting official research and independent organizations conducting research on birth and death events in Indiana. If you feel that data from Indiana would support your research, please submit a request for data:
- If you are a member of the media or are looking to submit a request specifically for public records, please submit a public records request.
- If you are seeking a birth or death certificate, please place a vital records order.

Vital Statistics result from analyses conducted on data collected from Vital Records. Birth, death, marriage, and pregnancy termination data are collected by the Indiana Department of Health's Division of Vital Records.
- Annual Reports are produced and published online for birth (natality), death (mortality), marriage, and pregnancy termination statistics.
