Consumer complaint sample(s) submitted for analyses will only be accepted through the Indiana State Department of Health Food Protection Division and/or other government agencies (Local Health Department).

Food products and some non-food products sold in retail establishments or restaurants can be Consumer Complaint sample(s). The sample(s) should be collected in its original packing material. If not, an appropriate sterile container should be used to collect the sample(s). A control sample(s) of the identical product with the same date and manufacturer code or prepared in the same day of the complain needs to be collected with the complaint sample(s).

For Sample(s) collection contact your Local Health Department or ISDH Food protection Program.

  • A sample(s) should meet all following criteria before it is accepted for analysis.
  • Sample(s) should be in original manufacturer’s packing material or an appropriate sterile container.
  • The sample(s) must be delivered in satisfactory physical condition and without any leakage.
  • A control sample(s) of the identical product with the same date and manufacturer code should be with the sample(s).
