Local Health Departments

The following resources are intended to aid local health departments in submitting quarterly reports, accessing immunization goals, increasing vaccination rates and more.

Reports

Quarterly reports are due 5:00 p.m. E.T. on the 30^th of the month following the end of each quarter. Please take note of your return code when submitting your quarterly evaluation report, as it will be needed when returning to update objectives/activities/outcomes. This code is unique to your individual survey response.

Goals

The Indiana Department of Health has identified the following goals for this grant period, along with resources to accomplish these goals:

1. Increase compliance with immunization recommendations in accordance with the Centers for Disease Control and Prevention’s 
2. Increase HPV vaccination of 11- and 12-year old’s
3. Increase seasonal influenza vaccination, with a special focus on 6 months to 9 years of age
4. Increase participation in the state immunization registry
5. Increase participation in MyVaxIndiana
6. Promote maternal and infant vaccinations as an essential component of the state’s strategy to reduce infant mortality
7. Identify under-vaccinated populations within the jurisdiction
8. Reduce vaccine wastage of publicly funded dose
