Medical Errors Reporting System

On January 11, 2005, Governor Mitchell E. Daniels Jr. issued Executive Order 05-10 requiring the Indiana Department of Health to develop and implement a medical error reporting system. The Executive Order cited successfully implemented medical error report systems for reducing the frequency of medical errors, revealing the causes of medical errors, and empowering healthcare professionals to design methods to prevent or discover errors before patients are harmed.

Consumer Complaints
Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.

Medical Error Reporting System
Indiana’s Medical Error Reporting System requires that hospitals, ambulatory surgery centers, abortion clinics, and birthing centers report any reportable event as defined by the rules that occurs within that facility. If you are a hospital, ambulatory surgery center, abortion clinic, or birthing center enter the reporting system to report an event.
