Help Me Grow is a system model that works to promote collaboration across child-serving sectors to build a more efficient and effective system that promotes the optimal healthy development of young children.

Help Me Grow is not a stand-alone program; it is a system approach that strengthens what already exists, collaborating partners together around shared goals, communities can avoid duplication, build existing resources and ensure children get the right support at the right time.

Think of the network of services for young children as a power grid. Help Me Grow makes sure the grid runs smoothly so families can plug in and get what they need, when they need it.

Help Me Grow’s Four Core Components Define the Model.

Centralized Access Point

A Centralized Access Point assists families and professionals in connecting children to the grid of community resources that help them thrive.

Family and Community Outreach

Family & Community Outreach builds parent and provider understanding of healthy child development, supportive services available to families in the community, and how both are important to improving children’s outcomes.

Child Health Provider Outreach

When potential concerns are spotted early on, they can be easier to address. Child Health Care Provider Outreach supports early detection and intervention efforts and connects medical providers to the grid of community resources to best support families.

Data Collection and Analysis

To make sure the resource grid is working effectively, Data Collection & Analysis supports evaluation, helps identify systemic gaps, bolsters advocacy efforts, and guides quality improvement.

It is the cooperation of these four Core Components that characterizes the Help Me Grow system model.

What Does Help Me Grow Do?

  • Assists families, primary care providers, and other community-based providers, including home visiting services, in identifying developmental or behavioral concerns in children.
  • Identify gaps in services.
  • Connect families with specialized programs and services focused on their needs.
  • Offer free Ages and Stages screening to open the door to your child’s development and wellbeing. Results of the screening will be shared with your family, and if needed appropriate referrals to local services and support will be provided.
  • A Care Coordinator will follow up and discuss previous referrals and will continue to support your family’s well-being.
