Health Facilities Consumer Reports
- ISDH Health Care Facility Consumer Reports - In 2003 the IDOH created its first health care facility consumer report. That report was for nursing homes. The IDOH has since added additional consumer reports for other provider types. Each consumer report provides information about health care facilities and entities. The information includes location, contact information, administrators, services provided, and survey history.
Medical Error Reports
- Indiana Medical Error Report - The Indiana Medical Error Report provides the number of reported serious adverse events at hospitals, ambulatory surgery centers, abortion clinics, and birthing centers. The report is based on the National Quality Forum’s Serious Adverse Events. The report is produced annually.
