Health: CDPC: IN-WISEWOMAN Eligibility

To be enrolled in the IN-WISEWOMAN Program, you must meet the following requirements:

Female;

Currently enrolled and remain eligible to participate in the Indiana Breast and Cervical Cancer Program (IN-BCCP);

In the priority age range of 40-64 years old;

Income at or below 200% of the federal poverty line;

Underinsured or uninsured; and

Unable to pay the premium to enroll in Medicare, Part B (if eligible for Medicare).
