Participant Self-Referral OR CJP Submission
Step 1: Submit the referral at pacerecoveryworks.org
Step 2: Liaison will reach out to the participant at the specified time
Step 3: Liaison will send a Referral Opportunity to up to 3 providers
Step 4: Liaison will send Intake Form to the allocated provider
Seeking services while incarcerated (including Works Release facilities)
Facilities that have approved providers entering jail or prison to deliver services may complete an Incarcerated Services Form. Once the form is approved, all requested referrals for services will automatically default to the designated in house service provider(s).
Referral Process

Step 3: Liaison will send referral to all providers approved for incarcerated services
