Indiana’s Healthy Food, Healthy Life Survey
Indiana SNAP recipients are invited to take a quick, confidential 10-minute survey to help the Indiana Family and Social Services Administration understand how SNAP benefits are used and how nutrition programs can better support families and communities across the state.
The survey will ask about the foods and drinks you buy, your eating and drinking habits, and how you feel about your health and well-being. You can also share any changes you’ve noticed in where or how you shop.
Your input will help the state understand how Hoosiers purchase food in different categories such as soft drinks, candy, fruits, and vegetables.
Your responses are anonymous and results will only be shared in summary form—never connected to you personally.
