Report Fraud
If you suspect fraud, waste, abuse or noncompliance with local, state or federal laws, the following Indiana state agencies provide secure reporting options:

  • State Board of Accounts (SBOA)
    What to report: Misuse of public funds, financial irregularities and noncompliance involving local and state government units. 
    How to report: File a confidential report using the SBOA Fraud Reporting Form: in.gov/sboa/about-us/contact-us/fraud-reporting-form/
  • Indiana Inspector General (OIG)
    What to report: Fraud, waste, abuse, mismanagement, ethics violations and wrongdoing within Indiana state government agencies and by state employees. 
    How to report: Submit a complaint through the Office of Inspector General: in.gov/ig/hotline/
  • Indiana Attorney General – Medicaid Fraud Control Unit (MFCU)
    What to report: Medicaid provider fraud, patient abuse or neglect in Medicaid‑funded facilities and misuse of Medicaid resources. 
    How to report: File a Medicaid fraud complaint through the Attorney General’s online portal: in.gov/attorneygeneral/about-the-office/medicaid-fraud-and-patient-abuse/
  • Indiana Department of Revenue – Tax Fraud Unit
    What to report: Tax evasion, improper tax filings, stolen identities used for tax purposes and other violations of Indiana tax law. 
    How to report: Submit a tax fraud tip through the DOR’s online reporting system: in.gov/dor/resources/fraud-prevention/report-tax-fraud/
