Digital certification of a license is used as proof of licensure when applying for a license in another state.
An electronic certificate will be provided to you via email after your request is received and processed.

(NOTE: The process for providing official certification changed as of July 1, 2023. This request system no longer auto-generates a digital certificate. The certification must now be processed by VetBoard staff. Please allow 3 to 5 business days for processing. If you have not received the certificate via email in that time, contact the Indiana Board of Veterinary Medicine.)
To obtain a digital certification, complete the following request form. Full payment (Visa, MasterCard, Discover or e-check) will be required before submitting. No refunds will be provided.

- Free Print My License
Log into MyLicense One to print a free copy of your license. Select the three dots under “Actions” beside the license you wish to print, then select “Print license,” which allows you to download or print a free copy of your license.
