Verification- This is the verification of an Indiana license to another state. It is an official statement of the basis of licensure and includes the licensee's name, profession, license number, licensure status, professional examination results, and any other conditions or restrictions on the license.
These verifications will be issued only at the request of the licensee for submission to another licensing authority. Verification requests must be in writing. There is a $10.00 fee for this service. Pelase be sure to include your name, profession, license number, date of birth, telephone number, and the address of the licensing authority to which the verification should be mailed, as well as any forms which must accompany the information.
Replacing lost or destroyed pocket cards- To replace a lost or destroyed pocket card, please write to the Professional Licensing Agency at 402 West Washington Street, Room W072, Indianapolis, Indiana 46204 and indicate that the pocket card has been lost, stolen, or destroyed. Include your name, address, license number, profession, date of birth, and social security number.
Replacing lost or destroyed wall certificates- To replace a lost or destroyed wall certificate, please make your request in writing to the Health Professions Bureau at 402 West Washington Street, Room W072, Indianapolis, Indiana, 46204. In the request, please include your name, address, license number, profession, and social security number along with a $10 fee for replacement of the wall certificate.
