Dietitians Licensing Information

Application Instructions

  • Application Information
    1. Completed Application  - Applications may be submitted online at MyLicense.IN.gov.
    2. Application Fee - $20 application fee to be paid by credit or debit card for applications submitted online.  All application fees are nonrefundable.
    3. Criminal Background Check
    4. Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s). Please upload at the time of application or log back into your account and use the License Update option.
    5. Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.  Please upload at the time of application or log back into your account and use the License Update option.
    6. Official Transcript - Submit official transcripts from your college and/or university from where you received your degree. Please upload at the time of application or log back into your account and use the License Update option.
    7. Pre-Professional Experience - If you are registered with the Commission on Dietetic Registration (CDR), please submit a copy of a current registration card that indicates the period of registration and expiration.  Please upload at the time of application or log back into your account and use the License Update option.
    8. Examination - The Commission on Dietetic Registration (CDR) has contracted with the American College Testing Program (ACT) to administer the Registration Examination for Dietitians. The examination is computerized and currently given year round, Monday through Friday. Cost of the examination payable to the CDR in the form of a certified check, cashier's check or money order.  See their website for the exact cost.     9. Verification of State Licensure(s) - Verification of any registration/license/certification to practice any health-related profession or occupation in another state or territory.  Verifications must be submitted directly from the state of issuance. Official electronic verification will also be accepted.

    The Fair Information Practice Act:  In compliance with Ind. Code 4-1-6, this agency is notifying you that you must provide the requested information, or your application will not be processed. You have the right to challenge, correct, or explain information maintained by this agency. The information you provide will become public record. Your examination scores and grade transcripts are confidential except in circumstances where their release is required by law, in which case you will be notified.

    Mandatory Disclosure of U.S. Social Security Number: Your social security number is being requested by this state agency in accordance with Ind. Code 4-1-8-1 and 25-1-5- 11(a). Disclosure is mandatory, and this record cannot be processed without it. Failure to disclose your U.S. social security number will result in the denial of your application. Application fees are not refundable.

    Abandon Applications:  If an applicant does not submit all requirements within one (1) year after the date on which the application is filed, the application for licensure is abandoned without any action of the Board. An application submitted after an abandoned application shall be treated as a new application.

  • Information Regarding Reciprocity
    • Licensure by Reciprocity: The Board shall issue a license to an applicant if the applicant satisfies the following conditions:
      • Holds a current license from another state or jurisdiction; and
        • that state's or jurisdiction's requirements for a license are substantially equivalent to or exceed the requirements for a license of the Board; or
        • when the person was licensed or certified by another state:
          • there were minimum education requirements in the other state or jurisdiction;
          • if there were applicable work experience and clinical supervision requirements in effect, the person met those requirements to be licensed in that state; and
          • if required by the other state or jurisdiction, the person previously passed an examination required for the license or certification.
      • Has not committed any act in any state or jurisdiction that would have constituted grounds for refusal, suspension, or revocation of a license, certificate, registration, or permit to practice that occupation in Indiana at the time the act was committed.
      • Does not have a complaint or an investigation pending before the regulating agency in another state or jurisdiction that relates to unprofessional conduct.
      • Is in good standing and has not been disciplined by the agency that has authority to issue the license or certification.
      • If a law regulating the applicant's occupation requires the board to administer an examination on the relevant laws of Indiana, the Board may require the applicant to take and pass an examination specific to the laws of Indiana.
      • Pays any fees required by the Board for which the applicant is seeking licensure.

Renewal Instructions

Renew your license online using mylicense.in.gov.

  • Renewal Information
    • Dietitian licenses expire on December 31 of even-numbered years.
    • The dietitian license renewal fee is $20.00 if paid before December 31. Renewals postmarked after December 31 will be assessed an additional $10.00 late fee.
    • Licensed dietitians are required to complete continuing education for renewal. For more information regarding CE please visit our Resources page.
    • Inactive renewals are now available online. Continuing education is not required for inactive renewal.
    Renewal notices are sent approximately ninety (90) days prior to the expiration date. License holders with valid email addresses on file will be emailed the renewal notice. Those who do not have valid email addresses on file will be mailed the license renewal notice; this notice is mailed to the address of record with the Board. The Board has no way of knowing whether or not a notice reaches its destination; therefore, when a notice has been emailed to a valid email address or mailed, the duty of the Board has been performed.
  • Reinstatement Information

    If your license has been expired for three or more years, you must reinstate your license to practice.  Please submit your reinstatement online with the additional documentation:

  • Renewal Documents

    To renew your license by paper you must select a document below. Print and mail the completed document with the required renewal fee to the Indiana Professional Licensing Agency. The address is on the form, and checks should be made payable to 'Indiana Professional Licensing Agency.'

  • Continuing Education Requirements

    (a) The amount of continuing education required for renewal of certificate is determined by the date on which the applicant for renewal initially received the certificate as specified as follows:

    • (1) An applicant for renewal who initially received certification within twelve (12) months of the expiration date is not required to complete a continuing education requirement.
    • (2) An applicant for renewal who initially received certification greater than twelve (12) months but less than twenty-four (24) months prior to the expiration date must provide proof of at least fifteen (15) hours of continuing education. The fifteen (15) hours of continuing education must be obtained subsequent to the date the applicant initially received certification but prior to the expiration date.
    • (3) An applicant for renewal who initially received certification more than twenty-four (24) months before the expiration date must provide proof of at least thirty (30) hours of continuing education during the two (2) years immediately preceding renewal.

    (b) The board shall determine which activities meet the continuing education requirements and may accept those courses approved by the CDR for this purpose. Applicants will be subject to random audit and must be prepared to submit documentation of completion of approved hours if requested by the board.
