The Indiana Professional Licensing Agency is proud to announce the creation of a new tool to help Hoosiers. This portal will allow Hoosiers to submit complaints via webform to be reviewed by our Office of Litigation Management.

Instructions:

  • Click here to begin the complaint submission process.
  • You will be taken to a new page to begin the webform.
  • Please follow any on screen instructions and provide accurate and up to date information.
  • When submitting a request form, you will be able to save your progress and resume at any time.
  • Upon completion of the form, the Office of Litigation Management will receive an email detailing your complaint and any files you have uploaded.
