Dual Eligible Special Needs Plans serve members who are eligible for both Medicaid and Medicare. D-SNP health plans offer care coordination and provide supplemental benefits to their beneficiaries. D-SNP-eligible individuals have the option to enroll in a D-SNP health plan during their Medicare annual election period or quarterly during the special enrollment period.
Not all D-SNP plans in Indiana serve the entire state. To determine individual service areas, please call the health plans' customer service numbers provided (see below).
Medicare Advantage organizations interested in operating a D-SNP plan in Indiana should contact Andrew Bean at Andrew.Bean@fssa.IN.gov. Health plans must contract with the Indiana Health Coverage Programs and hold a contract with Medicare.
