Dual Eligible Special Needs Plans

Dual Eligible Special Needs Plans serve members who are eligible for both Medicaid and Medicare. D-SNP health plans offer care coordination and provide supplemental benefits to their beneficiaries. D-SNP-eligible individuals have the option to enroll in a D-SNP health plan during their Medicare annual election period or at the start of every month.

Not all D-SNP plans in Indiana serve the entire state. For individual service areas, please contact the plans at their customer service numbers provided.

Health plans interested in operating a D-SNP plan in Indiana should contact Andrew Bean at Andrew.Bean@fssa.IN.gov. Health plans must contract with Indiana Health Coverage Programs and hold a contract with Medicare.

Please be aware of the following general timelines for applying or renewing a D-SNP contract with the state of Indiana.

  • November 2019: Notice of intent to apply from D-SNP applicants due to CMS for CY 2021
  • March 2020: Organizations should contact the state of Indiana to begin the DSNP contracting process
  • July 2020: Organizations submit signed state Medicaid agency D-SNP contracts to CMS

In Indiana four Dual Eligible Special Needs Plans serve the Medicaid and Medicare dually eligible population. To learn more about their programs, please reach out to each Health Plan’s customer service department. To enroll, eligible individuals can reach out to a specific health plan or contact Medicare at 800-MEDICARE.
