Vital Records

Birth/Death Certificates

Shelby County Health Department Vital Records maintains all birth and death records for Shelby County Indiana from 1882 to the present date.  Records prior to this date are not available.  Certified copies can be obtained in person at our office or by mail in request.  We are unable to take requests for birth or death certificates over the phone or via the internet.
We accept cash, money orders, debit or credit cards (service charge) in our office.  When ordering by mail we accept cash, and money orders made out to the Shelby County Health Department.
Mail order must include:
  • Birth or Death Certificate Application
  • Appropriate payment by Money Order or Cash
  • One ID needed:  Driver's License, Military ID, State ID care or Passport.
  • A self-addressed stamped envelope
  • Phone number and/or email address

Other Legal Documents

Appointments are required for paternity affidavits, paternity upon marriage, home birth registration, and amendments to Birth Certificates.

For marriage records, contact the Shelby County Clerk's Office at 317-392-6320.  They are located in the courthouse at 407 S. Harrison St. Shelbyville, IN 46176.

Delayed Registration of Birth is done at Indiana State Department of Heatlh.  Phone 317-233-1325
