the issuance of the requested records.

** Many birth and death records before 2007 may not be found in the digital record database. Within the handwritten records, information may be incomplete or hard to decipher. Please allow 7 days to receive your information **

- $5.00 fee in addition to the cost of the certificate.
**Genealogy fee ($5) will be due upon receipt of this application. Please note that we may not have the records, in that case the fee is non-refundable. Please allow 30 days to receive your information. **
