Applications for food permits

A Food Permit is required by law for any person/establishment that prepares or handles foods that will be sold to the public. See Indiana State Department of Health Rule: 410 IAC 7-24, 410 IAC 7-22, and Orange County Food Establishment Ordinance 2007-10, and 2014-5.

CHOOSE FROM THE FOLLOWING PERMIT APPLICATIONS
APPLICATION FOR RETAIL FOOD ESTABLISHMENT PERMIT FEES APPLY: Annual Fee $100 Fee is non-refundable. All permits expire December 31st of each year.
APPLICATION FOR SEASONAL FOOD VENDORS FEES APPLY: PER SEASON/ PER STAND $50 Fee is non-refundable. This permit is good for 6 months or 180 calendar days.
FESTIVAL FOOD VENDORS APPLICATION FEES APPLY: PER EVENT/ PER STAND $20 Fee is non-refundable. Permit is valid for one specified event and valid for no more than 15 consecutive days. This permit is good for any festival in Orange County.
