Click to Schedule your COVID Vaccine!

COVID Vaccine is now available for anyone age 5 years and older

  • Ages 5 and older are eligible for the Pfizer vaccine
  • Ages 18 and older are eligible for the Moderna, and Johnson & Johnson vaccine
  • Pfizer requires 2 shots separated by 3 weeks with a recommended booster 5 months following the second vaccine (booster only approved for those 12 and older)
  • Moderna requires 2 shots separated by 4 weeks with a recommended booster 5 months following the second vaccine (booster only approved for those 18 and older).  The booster shot is a half dose of the original vaccine.
  • Johnson & Johnson requires one shot with a recommended booster in 2 months (booster only approved for those 18 and older)
  • Booster doses can be mixed and matched but the original two-dose series should match
  • Those with moderate to severe immunosuppression should receive a third dose of Pfizer or Moderna 4 weeks following the second dose and a booster dose 5 months following the third vaccine.

**COVID Vaccines can now be administered at the same time as routine vaccines. Children ages 5-17 years can receive a COVID vaccine at the NCHD immunization clinic at their regularly scheduled vaccine appointments.

Lost COVID-19 Vaccination Card Replacement

COVID-19 vaccination records can be accessed at the Indiana Vaccination Portal at Access Indiana.  You will need to register for an account in order to access the record.  You may also come to the Noble County Health Department clinic during regular business hours.  You will need to present ID and a replacement card will be made for you for a $2.00 fee.

Tuesday and Thursday 8:30 am-11:00 am

Wednesday: 1:30 pm-3:30 pm
