Childhood Immunizations

The Health Department provides immunizations at NO COST for children who are on Medicaid, who are uninsured, underinsured, or an American Indian/Alaskan native. We can also bill most insurance companies for childhood immunizations. We administer vaccines for children (birth through 18 years) by appointment, Monday 9:30 AM to 5:00 PM and Tuesday through Thursday, 8:00 AM to 3:30 PM. Call 812-738-3237, Option 1, to make an appointment. Office hours are Monday 9:00 AM to 5:30 PM, and Tuesday through Friday, 7:30 AM to 4:00 PM.

Any shot received at the health department is recorded in the Children & Hoosiers Immunization Registry Program (CHIRP), which is accessible to qualified personnel from the internet at any time. Doctor's offices have access to CHIRP, as do all other Indiana county health departments.

NOTE: To bring a child for immunizations, you must be their parent or legal guardian. If you are not (if you are a step-parent, grandparent, babysitter, or another relative), you MUST bring a note signed by the parent or guardian stating that it is okay for you to bring the child for their immunizations.

Also, bring an updated copy of the child's shot record from the last place the child received vaccines and an insurance card for those who are insured.
