Question: What do I need to box in Indiana?
Answer: An Indiana Boxing License, a Federal ID and a HIV test results taken within 1 year

Question: Where do I get an Indiana Boxing License and a Federal ID?
Answer: You may obtain the application for an Indiana Boxing license or Federal ID either at a boxing event, or at the Indiana Gaming Commission, 101 W. Washington Street, East Tower Suite 1600, Indianapolis, IN 46204.

Question: Do I need to provide identification to receive a boxing license?
Answer: Yes. You may provide one of the following: (1) a birth certificate (2) baptismal certificate (3) passport (4) school records (5) DD214 from the armed services, or (6) a driver's license.

Question: Where may I obtain an application?
Answer: You may request one by calling our office at (317) 234-7164 or emailing us at iac@igc.in.gov; you may pick up one from the Indiana Gaming Commission at 101 W. Washington Street, East Tower Suite 1600, Indianapolis, IN 46204; you may apply at a boxing event, or download an application from our website at License Application and Instruction.

Question: How much does a license cost?
Answer: A physician, trainer, boxer, judge, timekeeper, and seconds license fee is ten ($10.00) dollars; a promoter license fee is one hundred ($100.00) dollars; a matchmaker and manager license fee is twenty-five ($25.00) dollars; a referee license fee is fifty ($50.00) dollars.

Question: Do I have to give my Social Security number on the application?
Answer: Yes. Your Social Security number is required under IC 4-1-8-1.

Question: How much does a Federal ID cost?
Answer: The original Federal ID is supplied at no cost and is good for four (4) years. A fee will be assessed for a replacement federal ID card.
