Apply/Renew/Edit Surplus Lines License

Initial Licensing Procedures

An application for surplus lines producer license may be submitted either through:

The applicant may choose to submit surplus lines application through the uniform NAIC paper application. Applicants must submit the following requirements.

Individual Paper Application

  • Completed, originally signed and dated application
  • Certification letter if your state does not participate in the NAIC’s producer database (Non-Resident Only)
  • Check made payable to Indiana Department of Insurance in amount of
    • Resident $80
    • Non-Resident $120, unless a retaliatory fee applies to your state
  • Resident surplus lines applicants must include an original passing ‘Score Report’ from your examination
  • If one or more of the background questions is answered “YES”, additional documentation is required. Please mail your documentation to the below address.

Business Entity Paper Application

  • Completed, originally signed and dated application
  • Certification letter if your state does not participate in the NAIC’s producer database (Non-Resident Only)

Renewal Procedures

License Renewal/Renewal Notice

IDOI emails producer renewal invoices to the producer's email address on file. Please note there is a $100.00 penalty for failure to report a change of name or address within 30 days of the change. If for some reason you do not receive a renewal invoice, it is still your responsibility to renew your license. If you do not receive an invoice within 90 days prior to the license expiration date you may go online to renew at either:

Renewal notices are available approximately 90 days before the license is due to expire. There are no continuing education (CE) requirements for Surplus Lines; please disregard the CE portion of the renewal form. Although not preferred, a producer may renew the license by submitting a uniform NAIC paper renewal application along with a check or money order made payable to Indiana Department of Insurance. The renewal application and fee may be mailed to the following address.

You will need to print your renewal license at www.Sircon.com/Indiana for an electronic processing fee of $5.15.

Renewal Date/Duration

License renewal dates will be the same as the applicant’s birth month. This change will be implemented on the applicant's next renewal date. A license issued or renewed after July 1, 2012, is for a two-year period.
