Thank you for choosing Indiana Women, Infants, and Children (WIC) to support your family. We are excited to have you join us at our special certification event. This event allows you to be seen sooner than your current scheduled appointment.

Below is important information you need to know:

WIC Staff

Who:  This event is for women who are pregnant or who recently had a baby, as well as infants younger than 1 year old and their siblings under age 5.

What:  This walk-in event is to enroll families in the Indiana WIC program.

When:  This event will be held over three days.  You can walk-in during any of the days and times below!

  • April 4, Noon – 7 p.m.
  • April 5, Noon – 7 p.m.
  • April 6, 8 a.m. – 4 p.m.

Where:  4150 N. Keystone Ave. Indianapolis, IN 46250

What should I bring?

  • Bring the person you want to enroll to the appointment with you.
  • Proof of identity. For example:
    • Birth Certificate
    • Driver’s license
    • Hospital Record
    • Immunization Record
    • Photo ID
    • Marriage license
  • Proof of address.  For example:
    • Bill or business letter addressed to you
    • Current driver’s license or photo ID with your current address
    • Rent receipt
  • Proof of income.  For example:
    • All paycheck stub(s) for the past 30 days
    • Award letter for alimony, child support or social security (including SSI)
    • Unemployment or worker’s compensation
    • Medicaid card
    • Award letter for TANF or SNAP

  • You can call the state WIC office at 1-800-522-0874 to learn more.
  • On the day of the event, if you have questions, you can call 211 ((866) 211-9966).

  • Lead and Healthy Homes

    Lead and Healthy Homes: The Lead and Healthy Homes staff will provide free lead testing to children and pregnant moms. No level of lead in the blood is considered safe. Studies have found that even small levels of lead in a child’s blood have been linked to lower intelligence in children, behavioral difficulties and learning problems.

  • Immunizations

    Immunizations: Immunizations have been shown to lower the risk of severe disease. Free vaccinations will be offered including:

    Pneumonia

    Pediatric hepatitis A

    Varicella (chicken pox)

    HPV

    Polio

    Meningococcal disease

    Pediatric hepatitis B

    Measles, mumps, rubella (MMR)

    Meningitis B

    Flu

    Tetanus, diphtheria, pertussis

  • Moms Helpline

    Moms Helpline: Moms Helpline team members will provide in person care coordination as well as free information about local resources, home visiting referrals, and other pregnancy and parenting needs.

  • Vital Records

    Vital Records: Proof of identity for the parent, guardian or caregiver, and the infant(s) or child(ren) applying for WIC is required. The office of vital records will issue standard birth certificates at no charge.

    Please note: Indiana does not issue birth certificates for individuals born in another state. Requests must be made directly to the vital records office in the state where the birth occurred.

  • HIV/STI/Viral Hepatitis

    HIV/STI/Viral Hepatitis: Knowing your HIV/STI/viral hepatitis status gives you powerful information to help you take steps to keep you healthy. The HIV/STI/Viral Hepatitis team will offer rapid testing for HIV, syphilis and hepatitis C as well as referral services.
