Genealogy Information and Process

Evansville Psychiatric Children’s Center medical records are maintained at the facility for at least 10 years, and until the former patient reaches the age of 23. Some older records are maintained at the Indiana State Archives in Indianapolis. For information on a deceased relative who was a former patient at EPCC, please contact our health information services director at 812-469-6800 x 4936. A Release of Information form will need to be signed to release records, along with photo ID and birth certificate of requestor and death certificate of former patient.
