Vaccinations and TB testing we prefer appointments but walk-in are welcome. Regular nursing hours are Monday though Friday. To schedule an appointment, please call our office at (812) 723-7112 ext 3.

WHAT TO BRING

• Identification (ie. Valid Drivers License or I.D. card with a photo and date of birth)
• Insurance Card
