This PHG web map serves as a single resource for finding health program districts and regions that serve each county.  Select a health program from the drop-down below.  Click or tap on map to display district information. Double-click or tap on map to display all districts for selected county.

Map Updates and Use

PHG collects and maintains information for health program districts and regions. If your program area is not included on map, please contact the ERC PHG program. Likewise, if information for your program area needs updated, please contact the ERC PHG program.

Program areas can link to or embed this map within their own webpage with only their districts displayed. In order link to this page and dictate the default district depicted on map, add the url parameter, 'Program', and assign the desired value from the list below.  Example:  http://www.in.gov/isdh/26777.htm?Program=0
