Below you will find a checklist of the items needed for your Unified Carrier Registration, USDOT Numbers, and/or Indiana Operating Authority.
Unified Carrier Registration (UCR)
New interstate carriers (crossing the state lines), brokers, leasing companies, and freight forwarders:
Existing interstate carriers (crossing the state lines), brokers, leasing companies, and freight forwarders seeking renewal:

New Intrastate (never leave Indiana) for-hire carriers (hauling someone else's property) or private property carriers of hazardous materials:

New Intrastate (never leave Indiana) private carriers (hauling your own private property) hauling nonhazardous materials:
Please note that there is not a renewal process if you are an intrastate private carrier hauling nonhazardous materials.
Indiana ID Numbers
Intrastate (never leave Indiana) for-hire carriers (hauling someone else’s property) with a gross vehicle weight rating or combined gross vehicle weight rating that is less than 10,000 pounds:Indiana Operating Authority
