Below is the "Application for Program Services". The Prosecutor's office will need to have you fill out this form to start enforcement of your case. Downloading this form and filling it out prior to going to the prosecutor's office will save time for all.
APPLICATION FOR TITLE IV-D CHILD SUPPORT SERVICES
IMPORTANT NOTE: Cases that receive a collection from the interception of the non-custodial parent's tax refund will be subject to the following fee per collection: $25 for federal offsets; 15% for state offsets. The non-custodial parent will, however receive credit for the full collection.
