These forms are for requests regarding IN.gov interactive services such as bug reporting or changes in applications.
Bug Report
This form is for reporting a bug or error in an existing interactive application.
Please expect a 2 hour response time Monday through Friday during normal business hours (8:00 am to 5:00 pm). For weekend and holidays please expect a 24 hours response and a 4 hour response during all other times.
Application Deactivation
This form is for requesting the removal of an existing interactive application.
Every effort is made to handle your request in a timely manner. At a minimum please expect this type of request to be addressed within 7 days from Thursday. Your requests will be prioritized based on the priority you assign. We appreciate your assistance in most accurately reflecting the priority level of your request.
Service Outage
This form is for reporting an existing service outage on IN.gov.
Please note: An outage is a complete lack of functionality. Users cannot access or use the application. A BUG occurs when the application can be accessed but an error occurs.
