Application Checklist

  1. Completed Application: Applications may be submitted online at MyLicense.IN.gov or completed by paper and mailed to our office.
  2. Application fee of $150.00: The fee is paid online during the online application process. For paper applicationsmake checks or money orders payable to ‘State of Indiana’.  All application fees are nonrefundable pursuant to statute.
  3. Criminal Background Check. This CANNOT be initiated until after your application and fee has been submitted.
  4. Juris Prudence Examination: Once the application has been made, the VetBoard will send you a short, Indiana-specific multiple-choice test. Be sure to complete and promptly submit the answer sheet according to instructions provided. The Juris Prudence examination is required only for first-time Indiana licensees.
  5. Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s). Please upload at the time of application or log into your account and use the “License Update” option.  Failure to upload positive response documentation will result in delays in the processing of your application.
  6. Name Change Documentation: Documentation of any legal name change is required if your name differs from what appears on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.  Please upload at the time of application or log back into your account and use the “License Update” option.  Failure to proof of name change documentation will result in delays in the processing of your application.
  7. Official Transcripts:  The transcripts from which you obtained your veterinarian degree must show that all requirements for graduation have been met and the date the degree was conferred. Please upload at the time of application or log back into your account and use the “License Update” option.
  8. Verification of Licensure: Verification of any registration/license/certification to practice any health-related profession or occupation in another state or territory.  Verifications must be submitted directly from the state of issuance.  Access the form via the link.
  9. Official Score Report: Applicants must submit an official score report of their North American Veterinary Licensing Examination (NAVLE). The score must be sent directly to the Board from the American Association of Veterinary State Boards’ (AAVSB) Veterinary Information Verifying Agency. Please contact the AAVSB for a “Score Reporting Service” form at the address listed below. This will explain how to obtain your scores and fee information.
  10. Continuing Education: First-time applicants do not have to report continuing education credits until the first renewal cycle.
  11. Controlled Substance Registration: Applicants who want a CSR must apply through MyLicense.IN.gov at the same time their veterinary license application is submitted. Failure to submit the CSR application at the time of submitting the veterinary license application will require the applicant to submit 2 hours of continuing education credits related to opioid abuse and prescribing to obtain and maintain the CSR.  Approved sources of opioid training may be found here.
  12. EDUCATION COMMISSION FOR FOREIGN VETERINARY GRADUATES (ECFVG) (if applicable) Applicants who have not graduated from an accredited college of veterinary medicine must submit satisfactory proof that they hold an Educational Commission for Foreign Veterinary Graduates (ECFVG) certificate issued by the American Veterinary Medical Association (AVMA) or a Program for the Assessment of Veterinary Education Equivalence (PAVE) certificate issued by the American Association of Veterinary State Boards (AAVSB). Applicants are not eligible for licensure until they submit satisfactory proof that they hold one of those certificates. Please contact ECFVG or PAVE on how to obtain certification. A list of accredited colleges of veterinary medicine is available on the AVMA website.
