A controlled substance registration (CSR) must be obtained directly through the Indiana Professional Licensing Agency (PLA), which issues that specific license in partnership with the Pharmacy Board.
Practitioners must hold a CSR in order to prescribe, administer, store, dispense or otherwise handle controlled substances in the State of Indiana. One CSR is sufficient for a practitioner to prescribe controlled substances throughout Indiana. A separate registration is required for each location where a practitioner administers, stores or dispenses controlled substances, unless the practitioner does so under a facility's CSR in accordance with 856 IAC 2.
CSR-VETERINARIANS ONLY: IC 35-48-3-3 has been amended to provide that a person licensed to practice veterinary medicine in Indiana holding an Indiana controlled substance registration is not required to obtain a separate registration to possess or dispense controlled substances in the usual course of veterinary practice at a site, so long as the site is not a principal place of business or professional practice.
