1977 Fund: New Baseline Application Version

Indiana Pension Law requires INPRS and all medical authorities to review the 1977 Police Officers’ and Firefighters’ Fund (1977 Fund) application (the baseline application) for membership every five years. This is to ensure that all physical and mental testing standards for police officers’ and/or firefighters' eligibility are current.

This Indiana Code requirement was recently completed, and a new version (State Form 4928 (R-20 / 1-25)) of the baseline application has been approved by the INPRS Board for immediate use. The previous version of the baseline application will be accepted through April 30, 2025.  If the older version is submitted after April 30, 2025, it will be returned to your unit, and you will be required to resubmit the application using the updated version. During this transition, please do not combine the two versions of the applications. These submissions will not be accepted as the required exams vary by application and cannot be combined.
