Intercept provides an alternative pathway for youth experiencing behavioral health crises who would otherwise remain in emergency departments awaiting placement. The program engages youth directly at the point of crisis, including in hospital settings, and rapidly transitions eligible individuals to intensive, in-home and community-based services. By delivering immediate stabilization, comprehensive family-focused interventions, and ongoing support in a more appropriate setting, the approach reduces reliance on inpatient hospitalization and helps break the cycle of repeat emergency visits. This model alleviates pressure on hospitals, improves access to timely care, and ensures youth receive services in environments that better support long-term recovery and family stability. Our HIPAA-compliant telehealth model connects patients with board-certified urologists and advanced practice providers within 1-3 days, which eliminates the 3-4 month specialist wait. We resolve 60-70% of urology cases entirely virtually, keeping patients in their communities and removing the specialist transport burden rural Indiana residents currently face. Our clinical team brings over 200 combined years of urology-specific experience and we are a subspecialty platform, not a generalist telehealth service. We accept Medicare, Medicaid, and most major commercial insurance, with transparent and affordable self-pay options. When in-person care is needed, we coordinate referrals to trusted local specialists to ensure continuity of care. That "magic" occurs when an authentic CHW - who shares lived experience with the people they serve and brings traits like empathy, reliability, and non-judgement - develops a trusting relationship that helps clients achieve their goals. CHWs can move the needle on Indiana's rural health priorities including maternal and behavioral health and chronic disease management. Our approach is designed to make this magic repeatable and scalable. We provide evidence-based infrastructure to over 70 CHW employers in 20 states (including Indiana).
