Common application mistakes to avoid

As you renew your application for licensing, please refer to these frequently asked questions to help guide you and help you avoid some common mistakes that might delay the licensing process:

Ministry

Q: Who is required to sign the application?
A: The person who can make decisions for the church or religious organization. This may include the Pastor of the church or Board Officer of the church or religious organization. Signer of the application must be listed on the Board of Directors list. Please note: The Director of the child care may not sign the application unless they are an Officer of the Board and they must use that title.

Q: Who should sign the attestation statement?
A: The signer of the application, Pastor of the church or Board Officer of the church or religious organization.

Q: What other documents are required each year?
A:

1.For a church:

  • Letter of Determination of Federal Income Tax Exempt Status Under 501 (c)(3) code from IRS.  If you are using your group’s 501(c)(3), a currently dated statement from the group leadership office stating that your Church is a member organization covered under the group ruling letter. This letter cannot be over 3 years old, or you can send in the Directory. Our office will need a copy of the directory cover page with a Date (not over 3 years old) and the page with your Church’s information highlighted.

2. For a Religious organization:

  • Letter of Determination of Federal Income Tax Exempt Status Under 501 (c)(3) code from IRS.
  • Article of incorporation from the State of Indiana. (We need all the pages.)

3.  For a church or religious organization:

  • Consent to release for background checks submitted to OECOSL for the applicant and all employees and volunteers
  • Fingerprints conducted through IDENTIGO for the applicant and all employees and volunteers prior to hire and then every three years

Licensed Center & Licensed Home

Q: Do I have to send in a business permit for each renewal application?
A. Yes, the business permit or a statement from the County is required for each renewal application. Please note: If your county/city/town does not require a business permit, please provide the letter from your county/city/town stating that a permit is not needed.  The letter must have a current date.

Q: Is it required for the same person who signs the application to also sign the attestation statement?
A: Yes, the same person must sign both documents.

Q: Do I have to send in my W-9 if I want to accept CCDF?
A: Yes, a W-9 is required for a program to accept CCDF funding. The W-9 is only required to be sent in once; it is not required during renewals. Note: The W-9 applicant must match your IRS filing.

Q:  What other documents are required each year?

Q: Can I choose more than one exemption on the License Exemption Affidavit?
A: No, you must match the exemption you chose on the application. You can only choose one.
