Program enrollment documents

- 1. Residency verification
- Provide a document, dated within the past 60 days, that includes your name, street address and city or zip code.
- Examples: Driver’s license, pay stub, lease, utility bill, or a piece of mail postmarked within the past 60 days.
- Provide a document, dated within the past 60 days, that includes your name, street address and city or zip code.
- 2. ID verification
- A picture ID is required for the applicant only.
- Examples: driver’s license, work ID, student or school ID.
- Provide verification of identity for all children and co-applicant, if present.
- Examples for children: birth certificate, hospital-issued certificate, or birth confirmation with date of birth listed.
- Examples for co-applicant: driver’s license, work ID, student or school ID, birth certificate, or vehicle registration.
- A picture ID is required for the applicant only.
- 3. Service need verification
If you are working for an employer,
- Provide one paycheck stub dated within the past 60 days.
- If you have a new job and haven’t received a paycheck yet, have your employer complete the new hire verification form.
- If paystubs are not available, or if you are paid by business/personal check, complete the Wage Detail form. Copies of the front and back of cancelled check(s) for each pay date will be needed.
- If you are paid in cash, have your employer complete the Cash form (OMWPK families only)
- If you receive tips on your paystub, complete the Tipped Employee worksheet with the paystub.
If you are self-employed, - Provide a profit and loss form for the previous calendar month AND provide a copy of your most recent tax transcript, which can be obtained online at www.irs.gov.
If you are looking for a job,
- Complete a Job Search form. (Only available to new applicants/co-applicants)
If you are attending training or education,
- If you are enrolled in college or trade school,
- Provide either your school schedule or a statement from the school on official letterhead with your name, school name, and semester dates.
- If you are attending middle or high school,
If you are a foster parent,
- Provide your Foster Care License and verification the child is a ward of the State.
- Examples: placement letter, per diem statement with child’s name, a letter from DCS caseworker.
These documents for your case should be uploaded to your online application under “document upload”
- 4. Child care provider information
- If you would like to do a search of eligible programs near you or on your route to work, please visit https://www.in.gov/fssa/childcarefinder/
- If you would like help finding an eligible On My Way Pre-K program, please contact your On My Way Pre-K County Project manager for help
- If you need help locating a CCDF approved child care provider, you may call Child Care Resource and Referral at 800-299-1627.
- Your chosen provider must complete the Provider Information form. Please upload this form to your online application.
- 5. Additional sources of income
- Please provide documents that show all your additional sources of income.
- Examples: current Social Security and/or SSI benefit letter, current TANF benefit letter.
- If you receive unemployment, go to the following website: https://uplink.in.gov/CSS/CSSClaimHomePage.htm. Find the document featuring the same date in your packet and print all unemployment amounts received. Please upload this document to your online application.
- If you receive Social Security Benefits/ SSI, please upload to your online application a copy of your current year benefit letter.
- If you receive TANF, you must upload to your online application a TANF benefit letter received within the last 12 months.
- Please provide documents that show all your additional sources of income.
- 6. Parent applicant worksheet
Complete the Parent/Applicant worksheet and sign the parent’s rights/ obligations on the last page. Please upload this form to your online application. (Electronic signatures accepted)
- 7. Attendance card agreement
Families must sign a Hoosier Works Child-Care Card form agreement as this will be what you use to log attendance at your chosen program. These will be used by both On My Way Pre-K families and CCDF families. Please upload your completed form to your online application.

Complete the On My Way Pre-K Parent Agreement and upload to your online application.
If any of the above documents for adults have a different or misspelled first or last name, complete a Name Attestation form and upload it to your online application.
