Community based service providers selected for RFP 10000108 contracts July 1,2015- June 30, 2017
Thank you to all providers who submitted proposals in fall 2014. Listed below are links to pdf documents by region indicating those providers in alphabetical order selected for a specific Community Based service in that region. These documents do not indicate a legal offer or contract but indicate those providers moving forward in the steps of contracting for Community Based Services. The providers listed will receive a letter from DCS indicating the next steps in the contract process.
