Life Insurance

Life insurance is something no one wants to think about but is important to have. The State of Indiana makes having coverage easier by offering five types of coverage: basic, supplemental, voluntary AD&D, spouse dependent life, and child dependent life insurance.

Newly eligible employees have a one-time opportunity during their initial eligibility period to elected guaranteed coverage without needing to go through Evidence of Insurability (EOI). Guaranteed coverage includes basic, supplemental and spouse dependent life insurance. Child dependent life and voluntary AD&D are always guaranteed meaning you may enroll in these coverages at any time if you are currently enrolled in basic life by contacting the Benefit’s Division at 317-232-1167 or toll free at 317-232-3011.

Evidence of Insurability (EOI)

Outside the initial eligibility period you may acquire or make changes to your life insurance plans by completing the Evidence of Insurability (EOI) process at any time throughout the year. Allowable changes include increasing your coverage level and/or adding an eligible spouse. This process applies to the basic, supplemental and dependent life policies.

It takes an average of 10 to 30 minutes to complete the online EOI application. Once submitted, Securian reviews your application and informs both you and Indiana State Personnel Department (INSPD) Benefits Division of its decision. If approved, INSPD Benefits will make the appropriate changes to your life insurance plans in PeopleSoft and start or change the premium deductions.

Beneficiary Changes

You may change your beneficiary information at any point during the year. For instructions on how to update your beneficiary information yourself, view the Self-Service Beneficiary Change - Quick-Step Guide. You may also contact the Benefits Hotline at 317-232-1167 or 877-248-0007 for assistance.

Filing a Claim

All claims must be reported to the SPD Benefits Hotline at 317-232-1167 or toll free at 877-248-0007 to initiate the claims process. Once a claim is reported, a Benefit Specialist will request the death certificate and submit the claim to Securian on behalf of the beneficiary. Even if you are not sure whether or not there was life insurance coverage you should contact the Benefits Hotline to ensure life insurance claims are submitted timely, if applicable.

Leaving State Employment

When you are no longer eligible for life insurance as an active employee, you may be able to elect to continue your coverage. Elections to port or convert must be made within 31 days of your active coverage terminating. To learn more or to request an election form please call 1-866-365-2374.
