Ten Indiana wireless telephone service providers take part in Lifeline, along with the landline service providers listed below.  

To apply for Lifeline benefits through a wireless provider, please contact the company directly. To apply through a landline provider, please complete this application and mail it directly to the provider. Completed applications must be submitted directly to your phone company.
