About the Residential Consumer Reports

The Indiana State Department of Health (ISDH), Division of Long Term Care, is pleased to provide consumers with a Residential Facility Consumer Report on each licensed residential facility in Indiana. The purpose of these Residential Consumer Reports is to provide information to consumers to assist in evaluating the availability and quality of residential facilities within the state of Indiana. Each Residential Consumer Report contains a profile of the facility, a report of the facility's recent survey history. The Residential Consumer Reports are limited to licensed residential facilities.

The ISDH cautions that no one tool or document can adequately assess the appropriateness of a residential facility for a particular patient. The information provided in the Residential Consumer Reports cannot substitute for a thorough review of the facility when choosing for yourself or a loved one. Consumers should also contact the administrators or staff for each residential facility they are considering and discuss the facilities and services available at that facility. Consumers are also recommended to spend time talking with medical professionals, family, friends, and consumer organizations about specific residential facilities and care provided by that facility.

Understanding the Residential Consumer Reports

Because survey results are technical in nature, they are sometimes difficult for consumers to interpret. In an effort to make survey results more available and understandable, the Indiana State Department of Health (ISDH) provides the following overviews of the survey process and general information about residential facilities:

It is important to note that the Residential Consumer Reports are updated every two weeks and placed on hold approximately two weeks before being published. The holding of the data for two weeks is required to insure that confidential survey information is not made public before it is legally allowed. The data contained in the Residential Consumer Reports is therefore approximately one month old.

In addition to being aware of the dates and the age of the data, it is important to understand that the data for every Residential Facility is at some point manually entered into a database which leaves the potential for human error. If an error was made by the ISDH, it will be corrected in the database and the Web page as soon as practicable. The residential facilities and the public are encouraged to notify the ISDH of any discrepancies found in the data presented in order to improve our overall data quality.
