Hospice Consumer Reports

The Indiana State Department of Health, Acute Care Division, is pleased to make hospice information available to consumers. This information is being made available to help evaluate the availability and quality of hospice programs within the state of Indiana. The Division inspects hospices to assess compliance with State Statute and Federal Conditions of Participation. This survey information, along with other information is being provided in a "Hospice Consumer Report" format. These reports may be used to compare information on licensed hospices.

About the Surveys:

 Surveys are unannounced

 Surveyors are registered nurses

A survey report is sent to the hospice program after each survey

If noncompliance with State Statutes and/or Federal Conditions of Participation are found in the survey, then the hospice is requested to submit a plan of correction to the Division

A follow-up survey may be made to verify that compliance has been achieved

Surveyors look at the care provided to a sample of the clients

Surveys only reflect a snapshot in time

About the Hospice Reports:

The consumer report has been developed to aid consumers in their choice of a hospice

The system uses existing data collected through the Division's surveys and data maintained in the Division's records

It also includes the number of full time employee equivalents, the date the agency was first certified, and whether or not Medicare and Medicaid are accepted. The report also identifies the counties that are served by this hospice.

The reports are generated twice a month and the statutory guidelines regarding the release of inspection reports will be observed.
