IN.gov Status

State Employees: If you have an IT issue that is not related to IN.gov (e.g., PeopleSoft, passwords, hardware/software, e-mail, etc.), please submit your ticket to the IOT Customer Service Desk so that your request is promptly resolved.

Project Change Order

This form initiates a change request for an existing interactive application.






Every effort is made to handle your request in a timely manner. At a minimum please expect this type of request to be addressed within 7 days from Thursday. Your requests will be prioritized based on the priority you assign. We appreciate your assistance in most accurately reflecting the priority level of your request.



Bug Report

This form is for reporting a bug or error in an existing interactive application.

Service Outage

This form is for reporting an existing service outage on IN.gov.

Please note: An outage is a complete lack of functionality. Users cannot access or use the application. A BUG occurs when the application can be accessed but an error occurs.
