Name and Address Change Information

Name Change

You may have the name changed on your licensure records by submitting a legal name change document (marriage certificate, divorce decree, social security card or other legal name change documents).  Go to MyLicense.IN.gov and log into your account to upload your name change documents.  Otherwise, you may submit your name change request with documentation via email at pla12@pla.in.gov or to our office as seen below:

Please include your name as it is listed now, your new name, and your license number or social security number on all correspondence.

Address Change

You are required to notify the Indiana Professional Licensing Agency of any address change. Please go to MyLicense.IN.gov and log in to update the mailing address on your licensure record.  Otherwise, you may submit your address change request via email at pla12@pla.in.gov or to our office as seen below:
