Plumbing Contractor Application & Instructions

Licensing Information

Apply online now! Go to MyLicense.IN.gov and you will either need to create or login to your Access Indiana, single sign-on account. You will complete the online application and submit payment with a credit or debit card. You will be contacted by a customer service representative with details of what additional documentation is required to complete your application. Applications are processed in the order received.

If applying through an Indiana Plumbing Commission Approved Apprenticeship School

  1. Must be at least 18 years old
  2. Completed Application: Applications may be submitted online at MyLicense.IN.gov or completed by paper and mailed to our office.
  3. Application Fee of $50.00 - Pay by credit or debit card for applications submitted online at MyLicense.IN.gov. If applying by paper/mail make checks or money orders payable to Indiana Professional Licensing Agency. All application fees are nonrefundable.
  4. Mandatory - Approved apprenticeship school section of the paper application completed with sponsor signature and current/active plumbing license.  The document may be uploaded, emailed, or mailed for processing
  5. Positive Response - If you have answered "yes" to questions 1 through 4, you must submit an affidavit detailing the occurrence/situation, the outcome, date of occurrence, if it is a malpractice payment the amount paid in your behalf.  If applicable, please submit copies of all court documents and/or arrest records. If you were ever placed on probation or are still on probation, you will need to submit order removing probation or setting terms of probation.  Letters from attorneys or insurance companies are not accepted in lieu of your statement.

If applying through Plumbing Trade (requires current out-of-state plumbing license)

  1. Must be at least 18 years old
  2. Completed Application: Applications may be submitted online at MyLicense.IN.gov or completed by paper and mailed to our office.
  3. Application Fee of $50.00 - Pay by credit or debit card for applications submitted online at MyLicense.IN.gov. If applying by paper/mail make checks or money orders payable to Indiana Professional Licensing Agency. All application fees are nonrefundable.
  4. Experience in Plumbing Trade - Out-of-State Licensed Plumber Section of the paper application completed by current/active plumbing license showing four (4) years of Experience in Plumbing Trade.  The document may be uploaded, emailed, or mailed for processing.
  5. If Applicant Does Not Have a License in Another State - The applicant's employer must submit a notarized Affidavit of Experience in Plumbing Trade (signature required).
  6. Positive Response - If you have answered "yes" to questions 1 through 4, you must submit an affidavit detailing the occurrence/situation, the outcome, date of occurrence, if it is a malpractice payment the amount paid in your behalf.  If applicable, please submit copies of all court documents and/or arrest records. If you were ever placed on probation or are still on probation, you will need to submit order removing probation or setting terms of probation.  Letters from attorneys or insurance companies are not accepted in lieu of your statement.

If applying through a Plumbing Business

**DO NOT COMPLETE “EMPLOYER AFFIDAVIT OF EXPERIENCE IN PLUMBING TRADE” IF YOU ARE APPLYING THROUGH A BUSINESS**

  1. Must be at least 18 years old
  2. Completed Application: Applications may be submitted online at MyLicense.IN.gov or completed by paper and mailed to our office.
  3. Application Fee of $50.00 - Pay by credit or debit card for applications submitted online at MyLicense.IN.gov. If applying by paper/mail make checks or money orders payable to Indiana Professional Licensing Agency. All application fees are nonrefundable.
  4. Experience in Plumbing Business – Non-licensed and nonregistered individuals completing the paper application must complete Section 4 – Experience in a Plumbing Business of the paper application.  The completed section may be uploaded, emailed, or mailed for processing.  The same information will be requested during the online process.
  5. Letter from employer on company letterhead/stationary – include job duties and dates of employment
  6. Positive Response - If you have answered "yes" to questions 1 through 4, you must submit an affidavit detailing the occurrence/situation, the outcome, date of occurrence, if it is a malpractice payment the amount paid in your behalf.  If applicable, please submit copies of all court documents and/or arrest records. If you were ever placed on probation or are still on probation, you will need to submit order removing probation or setting terms of probation.  Letters from attorneys or insurance companies are not accepted in lieu of your statement.

Examination Information

Candidates must have their application for examination reviewed and approved for eligibility by the Plumbing Commission before the candidate may register for the examination. Applicants may register for the examination on the Prove webpage: www.provexam.com.
