Fee Schedule

All fees should be paid by cash, check, or money order.  Checks and money orders should be made payable to the Indiana Professional Licensing Agency. We do not accept credit card payments, except for renewal applications completed online. 

Physician Application Fee:
$250.00 

Active Renewal of Physician License:
$200.00 every two years, due by October 31 of odd-numbered years

Inactive Renewal of Physician License:
$100.00 every two years, due by October 31 of odd-numbered years

Temporary License (M.D., D.O.):
$100.00

Limited Scope Permit Application Fee:
$100.00

Temporary Medical Permit (Teaching, Postgraduate, Non-ECFMG) Application Fee:
$100.00

Renewal of Temporary Medical Permit (Teaching, Postgraduate, Non-ECFMG):
$50.00

Reinstatement of physician license that has been expired for up to three years:
$250.00

Reinstatement of physician license that has been expired for more than three years:
$450.00
