Hoosier Healthwise and MED Works Premium Payments
Notice: Effective 09/01/2022, the following alternate formats will be available for monthly and premium change notice vouchers for Hoosier Healthwise Package C and MED Works:
- Braille
- Large font letters
- Audio CD

Monthly premium payments are required for Hoosier Healthwise Package C and MED Works. These members will receive a bill for the monthly premium that must be paid. Failure to pay monthly premiums may result in loss of coverage.
Members have several options for making these required payments:

Payment can be made using a credit card, debit card, or electronic check when calling or paying online. Your premium account number will need to be given for payment to be accepted. This can be found on your monthly voucher statement.

WHAT ARE THE PREMIUMS FOR HOOSIER HEALTHWISE PACKAGE C?
If your child qualifies for Hoosier Healthwise Package C, you will be required to pay a small monthly premium for services. The table below summarizes the monthly premiums based on the number of children you have and your monthly income.

If you qualify for MED Works, you will be required to pay a monthly premium for services. The table below summarizes the monthly premiums based your monthly income and whether you qualify as an individual (married or single) or qualify as a married couple (where both spouses qualify).

*If gross income is less than $1,518 for a single person or $2,058 for a married couple, you may qualify for Medicaid services with no premium.
