Care Coordination

The Porter County Health Department is dedicated to ensuring our community members and patients gain access to clinical care and connect with resources.

Our Care Coordinator can provide care navigation including but not limited to:

  • Bridge gap in care pathway
  • Connect to community-based resources & social services
  • Coordinate prevention screenings, training & education covered or provided by the PCHD
  • Care Transition
  • Self-management support
  • Referrals
  • Help to understand insurance benefits
  • Assist with finding a way to afford medication
  • Receive & process bicycle program applications
  • Coordinate & schedule Lyft rides provided by the health department
