The Noble County Health Department is partnering with St. Martin's Healthcare to enhance their reach to Noble County patients for the following programs:
Access and Linkage to Clinical Care
St. Martin's Healthcare offers care in the Noble County community through its mobile clinic. In office, they offer various health services: medical, dental, vision, and behavioral health services.
Tobacco Cessation Program
Patients receive education and are given tools to equip them on their cessation journey.
Chronic Disease Prevention
Patients with hypertension, diabetes, and hyperlipidemia with have lab results within certain ranges are offered the screening, prevention, and reduction program to prevent chronic disease.
