Family Court Quarterly Report form and Annual Report form
These report forms are a means for each court program to track its data. This data will be a primary factor for the Indiana Supreme Court to know which programs are the most effective and efficient. This information is critical and providing it will assist in determining which programs should continue to be supported. Please remember to review the Annual Report form early during the year so that you are aware of what data will be expected of your program at the end of the year.
Please read the instructions contained within each report and complete and electronically submit the Quarterly Report form to the Family Court grants manager at the end of each quarter (March 31, June 30, September 30, and December 31) and the Annual Report form as soon after the new year as is possible.
