Forms and Publications

The Annual Report on Evaluation of ADR Fund Plan forms are available below as Microsoft Excel documents. Either form is designed to be completed in Excel, saved, and then emailed to Mike Commons at michael.commons@courts.in.gov. You can print a copy of the form for your records, but you should submit an electronic file, not a printed file.

 ADR Fund Plan Annual Report - Contract Mediator
[Please use this form if: (1) the ADR Fund Plan utilizes contract ADR service providers to conduct the ADR services, (2) co-payments are not collected by the court/clerk, or (3) co-payments are collected by the court/clerk but are not deposited in the ADR Fund Plan account and are simply forwarded to the ADR service provider.]

 ADR Fund Plan Annual Report - Staff Mediator
[Please use this form if: (1) the ADR Fund Plan utilizes a staff member of the court to conduct the ADR services, or (2) co-payments are collected by the court/clerk and are deposited in the ADR Fund Plan account.]
